Health insurance and Billing

 

You will need to bring your insurance information with you at the time of your appointment.

White Oak Medical is an in-network provider for several health insurance companies:  Blue Cross Blue Shield, Aetna, Cigna and Connecticare.

If you’re unsure about your coverage for complementary and alternative medical care, contact your insurance plan prior to your appointment.

For patients without insurance, or whose insurance excludes coverage with White Oak Medical, payment is required at the time of treatment.

If you have out-of-network provider coverage, we can submit your claim as a courtesy.  But you will be responsible for payment at the time of your appointment.

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

We accept cash, checks, Visa and MasterCard.

Payment is due at the time service is rendered, unless other arrangements have been made in advance.

If you have questions regarding your account, please call us at 203.324.9239.

Any remaining balance after your insurance has paid is your responsibility.   You will receive a monthly statement.   Your prompt remittance is appreciated.

 

NOTE:  We offer a free 20-minute consultation to first time patients.  It is intended as an introduction only and does not include diagnosis or treatment.
